| Christiansburg Recreation Center Membership Application Form | This membership application must be completed and returned with two (2) proofs of residency, a recent photograph and membership dues (for non-residents/taxpayers) to the Christiansburg Recreation Center Supervisor and Equipment Station prior to use of facilities. No one will be allowed to use facilities without a completed membership application being on file and valid identification card. |

Membership Identification cards are made: Monday through Friday 8:00 AM until 6:00 PM Saturday 10:00 AM until 4:00 PM Sunday 2:00 PM until 4:00 PM |
